Forms

Throughout the year we will participate in activities that require parental permission. These include (but are not limited to):

• Overnight Camping 

• Hiking

• Day Trip Activities

The forms required for Cub Scouts to participate are as follows. Please print, fill them out and bring them to the committee chair ASAP: 

• Annual Health and Medical Information Form 

• Activity Consent Form 

• Photo Release Form
